It is of course, a quagmire. I mean there are so many laws in this country that are completely based on religion. Its sort of ironic that you can get a religious exemption from a law that was based on religious principles. Unless you really go libertarian on most things you keep the door open for religion and morality based laws, which can be good and bad. Specifically dealing with the denying medical coverage based on religion, it seems that you could make a strong equal protection argument under the 14th Amendment that should allow anyone to do it if the religious types are doing it.
